Camper Checklist

 

Step 1. Verify Church Registration

Check with your church to make sure your church is registered for camp. If your church has not registered, we will not be able to process your Medical Information & Release Form.

Step 2. Medical Information & Release Form

Parent/Guardian complete the Medical Information & Release Form online for each camper. If a camper begins medications after filling out the medical form, a new medical release form must be submitted. Parent/Guardian will receive a copy of submitted form via email. The Group Leader may ask for you to forward this to them to verify that it has been completed, and so they have a copy of the information while traveling to and from camp.

Step 3. Medications

  1. Medications MUST remain in original prescription bottle with the name of the camper. Medications must be dispensed per the instructions on the prescription bottle.

  2. Place all of the campers original prescription bottles in a ziplock bag and label the ziplock with the NAME OF CAMPER and NAME OF CHURCH.

  3. All Medications MUST be turned into the Group Leader prior to arriving. The Group Leader will turn in all medications for the group to the Camp Health Officer upon arrival.

Sponsor Checklist

 

Step 1. Verify Church Registration

Complete the Medical Information & Release Form online. The Medical Form must be completed by every Adult and Student who attends camp. You will receive a copy of submitted form via email. Step 3. Medications

  1. Medications are not allowed to be kept in the dorms. Adults taking medications will be required to turn in their medications to the Camp Health Officer to dispense.

  2. Medications MUST remain in original prescription bottle with the name of the camper. Medications must be dispensed per the instructions on the prescription bottle.

  3. Place all original prescription bottles in a ziplock bag and label the ziplock with the NAME OF CAMPER and NAME OF CHURCH.

  4. All Medications MUST be turned into the Group Leader prior to arriving. The Group Leader will turn in all medications for the group to the Camp Health Officer upon arrival.

Step 4. Child Protection Training

Complete the Child Protection Training course and take the exam. Upon completion, a certificate will be emailed to you. The certificate should be turned into your church office for them to keep on file.

Group Leader Checklist

 

Step 1. Register Your Church

Make sure that you have filled out the church registration form for the appropriate camp your church is attending. Registration will not be complete until the specified deposit has also been paid for each individual.

Step 2. Individual Registrations

Complete and keep up to date the Camper Registration Form that was shared with you from the camp director. This form may look a little different depending on the camp you are attending, but this is where you will list the names, grade level, and shirt sizes of all the Adult/Sponsors and Students attending camp with your church.

Step 3. Medical Information & Release Forms

All Adult/Sponsors and Students who attend camp are required to complete the Medical Information & Release Form online. The adult or parent/guardian completing the form will be emailed a pdf of their responses. You may want to ask them to forward the confirmation to you so you know it has been completed and you have a copy of their medical information if it is needed while traveling to and from camp.

Step 4. Sponsor Documentation

When attending summer camp each church must complete and bring the Sponsor Documentation Form which must be signed by church pastor or official representative who is not coming to camp, verifying each sponsor has:

All background checks and Child Protection Training Certificates should be kept on file with your church office. Only the Sponsor Documentation Form will be turned in to the camp.

Step 5. Medications

Prior to departing for camp, check all Adult/Sponsor and Student Medical Information & Release Forms and collect all camper medications that have been listed on their form. Medications must remain in the original prescription bottle. Each campers medications should be placed in a ziplock bag labeled with NAME OF CAMPER and NAME OF CHURCH. You will turn in all medications for your group to the Camp Health Officer upon arrival (for Summer Camps).

Step 6. Final Payment

Bring check for final amount owed to pay at check-in.
