SUMMER ONLY MEDICAL INFORMATION & RELEASE FORMS
Every Adult/Sponsor and Student/Child Camper must complete a Medical Information & Release Form to attend camp at Alto Frio.

The Medical Information & Release Form contains a section to report medically necessary dietary needs. To ensure appropriate accommodations are available, the Medical Information & Release Form must be completed at least one week prior to camp.
Campers who have reported a medically necessary dietary need on their Medical Information & Release Form can give their name and name of church to dining hall staff during meal times to receive their accommodated meal.
MEDICAL ACTION PLANS
Campers with a history of Seizures, diagnosis of Asthma requiring an inhaler, or an Allergy that may require an Epinephrine Pen, must also bring either a Seizure Action Plan Form, Asthma Action Plan Form or Allergy & Anaphylaxis Plan Form (whichever applies) signed by their doctor.
*If your child has one of these plans on file with their school nurse, you may request a copy from the school.*
